Members are required to specify their membership association and number on the registration form.

**  ESPR Young Investigator must be an ESPR member and under 35 years of age . Clear copy of ID indicating date of birth is to be sent to the Congress Secretariat  either as scanned attachment by e-mail: reg_eaps12@kenes.com or by fax to the attention of EAPS 2012 to number: +41 22 906 9140  
*** Residents/students must provide a signed document from their head of department on official letterhead to support their application for the reduced rate.
***** Nurses (EAP, ESPR & ESPNIC members) and Physiotherapists are required to send proof of status and valid membership number to the Congress Secretariat when registering.
****** Nurses (non-members) and Physiotherapists are required to send proof of status to the Congress Secretariat when registering.

PAYMENT

All participants must submit a completed registration form. Payment or proof of payment should accompany the registration. Please make a photocopy of your registration form for your own files.

Registration fees must be paid only in Euro (€)
Registration fees can be paid by the following methods: (in €)

• By credit card:  MasterCard / Visa / American Express (Diners Club cards may not be used for registration payment) 
  Your monthly credit card statement will indicate payment made to 'Kenes International Organizers of Congresses'.

Important: All bank charges are the responsibility of the participant and must be paid in addition to the registration fees.
Please remember to specify the name of the Congress and the participant on the bank transfer document.

If payment is made by a company or for more than one person, please make sure all names are indicated.
The completed registration form together with the relevant payment must be received by July 2, 2012, in order to benefit from the reduced registration fee. Thereafter the registration fees will increase as published.

CONFIRMATION

As soon as the Secretariat has received the complete registration form and the required payment, a written confirmation will be sent by e-mail.

REGISTRATION CANCELLATION POLICY

All cancellations must be faxed, electronically mailed or post-marked.
Prior to July 2, 2012- Full refund less €35 handling fees
From July 3, 2012 until September 20, 2012 - 50% refund
From September 21, 2012 – No refund

GROUP REGISTRATION

Group Registration procedure will be valid for 10 participants and more.
