membership for 2010 prior to registering to the Congress.

 **In order to obtain the special fee for Residents/Nurses an approval letter signed by your institutions’ Head of Department, together with a copy of a valid Resident/Nurse’s card, must accompany the registration form (if registered via the website,
please mail to reg_esra2010@kenes.com  or fax the letter to the attention of Valentina Schnee at +41 22 906 91 40). 

NB: online registration to Diploma Part 2 is available after completed registration to the congress

*** ATTENTION : Please note that the ESRA Diploma Part 2 will take place on Tuesday, 7^th of September 2010 between 09:00 am and 05:00 pm.

Registration will be confirmed only upon full payment. Unpaid registrations will not be valid.

Payment of registration fees (in €EURO) can be made as follows:

Please ensure that the name of the Congress and of the participant are clearly stated on the bank transfer to allow easy tracking.

Bank charges are the responsibility of the participant and should be paid at source in addition to the registration fees.

Registration will only be valid upon receipt of the full payment by the Registration Department according to the deadline indicated. An e-mail confirming registration will be sent after receipt of the required fees.

Outstanding payments will be collected on-site and charged according to the on-site rate. A copy of the bank transfer (or other proof of payment) will be required in the event that registration fees were not credited to the Congress account on time.

Registration Cancellation Policy:

• Cancellations received until June 16, 2010 – full refund
• Cancellations received  until and including August 18, 2010 – 50% will be refunded
• From August 19, 2010 onwards– no refund will be made.
