All participants including invited speakers must submit a completed registration form.

Payment Instructions

Payment of fees must be made in EURO and should be paid using one of the following options:

• Option 1: Credit card: VISA, MasterCard, Diners Club or American Express.
  (*Payments made with American Express will be charged in U.S. Dollars
  according to the exchange rate on the day of the transaction)
  Your monthly credit card statement will indicate payment made to
  'Kenes International'.

• Option 2: Bank Transfer: payable to “KENES INTERNATIONAL (EFIC 2009)",

  Account: 693980-52-138. Please make sure that the name of the Congress and of the participant is stated on the bank transfer.
  Bank charges are the responsibility of the participant and should be paid at source in addition to the registration and accommodation fees.

• Option 3: Cheque made payable to:  “EFIC 2009”

• Postal orders and travelers cheques will NOT be accepted.
  
  The completed registration form together with the relevant payment must be received by latest June 15, 2009, in order to benefit from the reduced early registration fee.
  Thereafter the registration fees will be as published.
  Registration may also be made online via the Meeting website.

Registration fees for EFIC Chapter Members & IASP® Members and Non-Member participants include:

Participation in all scientific sessions, congress kit, Final Programme, Abstract Book and/or CD-ROM other printed material of the Congress, an invitation to the Opening Ceremony and the Welcome Reception, as well as a two year subscription to the EFIC’s European Journal of Pain (EJP).

Registration fees for Nurses/Students/Trainees, Category A and Category B Participants include:

Participation in all scientific sessions, congress kit, Final Programme, Abstract Book and/or CD-ROM other printed material of the Congress, and an invitation to the Opening Ceremony and the Welcome Reception.

Registration fees for Accompanying persons' include:

An invitation to the Opening Ceremony and the Welcome Reception
Two half-day tours.

Registration Cancellation Policy

All cancellations must be faxed, electronically mailed or post-marked:
Refund of registration fees will be as follows:

• Until and including June 1, 2009– 100% refund (less € 35 handling fee)
• Postmarked from June 2, 2009 – 50% refund
• No refund on cancellations after July 30, 2009
